HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
We are required by law to maintain the privacy of protected health information, to provide individuals with notice of our legal duties and privacy practices with respect to protected health information, and to notify affected individuals following a breach of unsecured protected health information. We must follow the privacy practices that are described in this Notice while it is in effect.
Uses and Disclosures of Protected Health Information
We use and disclose health information about you for treatment, payment, and healthcare operations. For example, information obtained by a dentist, nurse, or other member of your care team will be recorded in your record and used to determine the course of treatment that should work best for you.
Your Rights
You have the right to inspect and get a copy of your health information, request corrections to your health information, request restrictions on how your health information is used and disclosed, receive a list of disclosures we have made, and request that communications with you be kept confidential.
